At an intersection you see a circular red light lit at the same time as a green (or amber) turn arrow, on the same signal head. Is this a malfunction?

Correct answer: A. No — a circular red may legally appear with an arrow indication; it's how a protected-turn phase is normally shown

Regulation 626 prohibits a circular red from being shown at the same time as a circular amber or circular green — but it specifically permits a circular red to be displayed at the same time as an arrow indication. This is the normal appearance of a protected-turn phase (common on combined "doghouse"-style heads): through traffic sees red while a turn arrow is separately green or amber for the protected movement. It's a legal, intentional configuration, not a fault.
